By starting early, we can establish a baseline. If any risks or needs are identified later, we’ll be able to offer proactive interventions to help you or a loved one stay safe at home.
If your results indicate any risks or unmet needs, a member of our Aging in Place Solutions team will contact you to review your report. We’ll also provide the right resources and create a customized action plan to support your aging in place goals.

Yes. In some cases, a caregiver or third party is the best person to complete the assessment. In others, the senior’s direct input is required. Either way, reviewing the assessment criteria helps families understand the factors that influence successful aging in place.
